Terms and conditions for Unlimited Stroller Membership

FIT4MOM SOUTHEASTERN CT MONTHLY MEMBERSHIP AGREEMENT
This contract is made between the client and FIT4MOM Southeastern CT. It is a month-to-month agreement and may be terminated by either party with the proper advance notice, except as provided in the initial 10-day cancellation policy below. This membership allows you to attend FIT4MOM Southeastern CT classes.

AUTHORIZATION FOR PAYMENT

I authorize FIT4MOM Southeastern CT to initiate a charge to the account entered into Pike13 for my membership dues. This authorization remains effective until I have canceled my membership in writing through the FIT4MOM Southeastern CT owner.

I have the right to stop payment on an Electronic Funds Transfer debit by notifying my bank. This, however, does not void my contract with FIT4MOM Southeastern CT to fulfill my payment commitment, and I am obligated to pay. Dues will be drawn monthly from the account on file on the first of each month. The processing date for debit cards may vary due to banking procedures. If charges are returned, they will be subject to a late fee. Members are responsible for updating their credit card information online. Members may not attend classes who have unpaid balances. If a membership remains unpaid, it will be canceled.

CANCELLATION POLICY
To cancel this membership, the member must contact FIT4MOM Southeastern CT at destinimcallister@fit4mom.com at least 30 days before their next payment due date to request a cancellation. Cancellations will be confirmed in writing to the member once processed. No refunds will be given for dues once it has been paid. This membership is not transferable to another person or anywhere outside the Southeastern CT franchise. Once a membership is canceled, if the member wants to re-enroll, they will be charged half the standard registration fee. Instructors are not authorized to cancel memberships.

HOLD POLICY
Memberships may be held for a maximum of 6 months per hold period. To place a hold on this membership, the member must contact FIT4MOM Southeastern CT at destinimcallister@fit4mom.com at least ten business days before their next payment due date to request a hold be placed on their membership. Holds will be confirmed in writing to the member once they have been processed. Hold starts will coincide with the following date dues are scheduled to be drawn out and can end on any day of the month. When a hold period is concluded, the monthly auto payments will automatically resume on the return date. It is up to the client to keep track of their return date. A $30 processing fee will be charged to your credit card on file for the hold period. Instructors are not authorized to place a hold on memberships.

FIT4MOM ONDEMAND MONTHLY SUBSCRIPTION

With your paid monthly membership, you are given free access to FIT4MOM OnDemand. You will have unlimited access to OnDemand workouts. When your FIT4MOM membership is canceled correctly according to the Terms and Conditions, your OnDemand subscription will be automatically canceled. If you want to keep your OnDemand subscription, you can re-enroll using the same login information and purchase your desired paid subscription.

INITIAL 10-DAY CANCELLATION POLICY
You, the buyer, may cancel this agreement at any time before midnight of the tenth (10th) calendar day after your initial payment transaction. To cancel this agreement, written notice must be received by FIT4MOM Southeastern CT within the initial ten days. You are responsible for paying for the classes already attended at the drop-in rate.

By purchasing this contract, you are agreeing to the terms of this contract. Terms of this agreement may change with the proper advance written notice.
